Primary Care Network Development
Supporting the four regional PCNs’ delivery of high-quality primary health care within Hambleton, Richmondshire and Whitby.
Primary care networks (PCNs) are groups of practices working together to focus on local patient care. Heartbeat plays a pivotal role in supporting the four regional PCNs’ delivery of high-quality primary health care within Hambleton, Richmondshire and Whitby.
The support is centred around business development and coordination to spot opportunities for greater collaboration and efficiencies. Working with the Clinical Director of each PCN, Shirley Moses and Jayne Bone from Heartbeat operationalise plans, ensuring national requirements are met, financial management is in place and shared learning takes place.
The Heartbeat team is also responsible for the recruitment and management of additional and added valued roles such as social prescribers and clinical pharmacists who work within and across a number of practices, providing their expertise and services to patients.
